Responsible for the development, monitoring and supervision of the rehabilitation/restorative program for the facility. Promotes a restorative nursing philosophy among all facility departments.





  • Complete initial rehabilitation/restorative program assessment to assign appropriate program placement
  • Initiate and update resident rehabilitation/restorative plan of care.
  • Serve as primary liaison with the licensed therapy staff
  • Coordinate the implementation of the rehabilitation/restorative program or recommendations made by the licensed therapy staff
  • Monitor all aspects of the nursing rehabilitation/restorative program on an ongoing basis
  • Review all rehabilitation/restorative aid documentation for accuracy and completeness, update as needed.
  • Assure licensed documentation in the medical record supports resident progress in their nursing rehabilitation/restorative program.
  • Communicate with nursing rehabilitation/restorative aides on a regular basis to review the status of individual resident programs, update programs, review charting and provide input to the aides regarding their skills.
  • Give input regarding the restorative needs of residents at the resident care plan meeting
  • Serve as a resource for other facility staff involved in the provision of restorative services (e.g. activities, social services)
  • Participates in staff education and orientation related to nursing rehabilitation/restorative philosophy and programs
  • Represent the nursing rehabilitation/restorative program to families, physicians, and other individuals involved in the resident’s care.
  • Contribute to rehabilitation/restorative aides annual reviews
  • Conduct periodic audits as assigned
  • Report to the CQI committee monthly
  • Ensures residents’ rights are respected.
  • Other duties as assigned.






  • Current license by the state Board of Nursing
  • May be LPN; prefer RN
  • Minimum one year experience as a licensed nurse in long-term care
  • Demonstration of clinical competency in nursing rehabilitation/restorative techniques.
  • Ability to communicate effectively both verbally and in writing
  • Willingness and ability to function as coordinator of restorative program and supervision of rehabilitation/restorative services
  • Ability to participate in physical return demonstration for the purpose of competency skills check
  • Creative problem solver
