Responsibilities

  1. Coordinate the RAI Process.
  2. Work in Collaboration with the interdisciplinary team to assess the needs of the patient/resident
  3. Significantly involved in Care Coordination meetings
  4. Ensures accurate and timely MDS assessments according to state and federal regulations.
  5. Ensures and completes accurate coding of the MDS assessment with information obtained via medical record review, observation and interview with center staff, patients, residents and family members.
  6. Maintains the tracking system of MDS assessment schedules (time frames and due dates).
  7. Coordinates Care Plan conferences with the interdisciplinary team, patient, residents and families.
  8. Obtain, review and maintain all State and Federal reports, making appropriate corrections timely.
  9. Monitors Quality Measures and ensures that MDSs are accurate to support and reflect the Quality Measures.
  10. Provides education related to the RAI Process
  11. Coordinates and completes electronic submission of required documentation to the State data base and other entities per company guidelines and State and Federal regulations.
  12. Ensures Medicare and Medicaid regulatory guidelines are completed accurately and timely (i.e.: certifications, denial letters, skilled documentation, coverage criteria, etc…)
  13. Facilitates daily care coordination meetings to ensure care resources are aligned with benefits and patient needs
  14. Participates and facilitates Engage/72 HR Admission meeting. Ensures coordination as part of the care management system
  15. Responsible for the coordination of the restorative nursing program including integration of the
    activity department
  16. Ensures timely communication with the patient/family regarding changes in condition and discharge planning
  17. Responsible for the ongoing communication to all insurance case managers
  18. Manages all initial reviews, interim reviews and discharge reviews (Managed Care
    authorizations/reauthorizations).
  19. Collaborates with clinical team in the development and execution of care plan
  20. Ensures timely facilitation of patient care rounds/conferences to review treatment plan and identify discharge and post discharge needs
  21. Ensures utilization of company tools/resources related to care management – e.g. Dart
    Chart/Managed Care Log
  22. Coordinates discharge planning and post-discharge communication - home health/DME, etc.
  23. Determines need for non-coverage notification and is responsible for issuance of the beneficiary
    notices as well as communication with patients/families/payers
  24. Monitors Quality Measures along with other members of care coordination team
  25. Serves as a skilled care documentation specialist ensuring that Medicare and Managed care
    regulatory guidelines are completed accurately and timely (i.e. certifications, denial letters, skilled documentation, coverage criteria, etc.)
  26. Reviews Additional Documentation Requests (ADRs) with Health Information Management
    Coordinator (HIMC) to ensure all documents are available and meet the request

Qualifications

  1. Two years of MDS experience preferred, but not required
  2. Must hold current registered nursing license in the state of employment. 
  3. Experience in Long Term Care preferred.
  4. Computer skills such as Data Entry and Word processing required. Working knowledge of the MDS 3.0 (current version of minimum data set).
  5. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to skilled nursing facilities.
  6. Must possess the ability to plan, organize, develop, implement and interpret the programs, goals, objectives, policies and procedures that are necessary for providing quality care.
