Responsibilities

 

 

KEY RESPONSIBILITIES: essential job functions to this position; management reserves the right to modify this list and other functions as deemed necessary

 

  • Coordinate all aspects of patient referrals and admissions to the Center, involving Center team members and utilizing strong office skills
  • Responsible for the training, leading, coaching and growth of direct reports
  • Accurately represent Center services and clinical capabilities, including appropriate Features-Benefits-Impact, effectively demonstrating tour best practices and use of Diversicare Sales Steps
  • Communicate effectively with patients and families to determine services and care that best meet their needs, for the right care in the right place at the right time 
  • Thoroughly educate patients and families and understand patient/family expectations to provide a positive experience and meet or exceed expectations
  • Participate in Center meetings and activities as required, and thoroughly and accurately complete and/or submit any required reports, paperwork, and documentation  Participate in marketing activity as determined with Administrator
  • Collaborate with referral sources and team members to facilitate open communication
  • Develop and maintain strong relationships with referral sources, uncovering needs and matching appropriate Diversicare solutions
  • Meet or exceed goals determined with Administrator
  • Exhibit leadership qualities, managing the admission process and collaborating with Center team
  • Maintain the highest standards of professionalism and service excellence with activity and behavior that supports the Diversicare Mission, Values, and Service Standards
  • Assists in pre-admission and admission process with customers; aids clients in adjusting to nursing home placement.

  • Interviews residents and families to obtain relevant past and present adjustments, behavior and lifestyle.

  • Documents progress and updates plans of care at the interdisciplinary care conference.

  • Arranges transportation, makes appointments and acts as a liaison between all departments, residents’ families and outside agencies.

  • Assists in handling and accepting placement through one-on-one counseling and group work.

  • Ascertains potential and develops discharge plans when clients are admitted.

  • Arranges for post discharge services and follow-up care.

  • Assesses emotional, financial, spiritual, educational and social needs of residents; ensures those needs are Manager, Education & Training by the facility or through the use of outside agencies.

  • Acts as resident/family advocate and ensure the resident is knowledgeable in and exercises his/her rights.

  • Responsible for organizing and conduction of Family Counsel Meetings and maintaining minutes of the meeting.

  • Maintains effective communication with residents, families, healthcare professionals and facility staff.

Qualifications

  • Bachelors Degree
  • Long term care / skilled nursing facility experience 
  • Experience in admissions or other related fields such as Social Work or Psychology
