Responsible for leading and optimizing the admissions program by overseeing inquiry, referral, and admissions processes; coordinating electronic customer systems; and developing workflows to ensure accurate utilization and adherence to company standards. Partners with regional teams and sales leadership to strengthen relationships, drive conversion targets, and support the company’s commitment to patients served.
KEY RESPONSIBILITIES: The following statements are intended to describe the general nature and level of work being performed and are not intended to be a complete list of all responsibilities, duties and skills required.
• Achieve budgeted patient served, maintain quality payer mix, and meet assigned conversion targets in alignment with departmental initiatives.
• Train, guide, and coach indirect reports to ensure admissions excellence.
• Coordinate and optimize use of electronic customer relationship systems (CRM and related platforms.
• Manage Diversicare user access and maintain accurate system profiles at both the center and company level.
• Cooaborate with cross-functional teams to align care options with patient needs.
• Build and maintain strong working relationships with qualified referral sources.
• Analyze referral and admission trends to identify opportunities for growth in patient types served.
• Track referral and admission activity through PCC, CRM, and other designated systems.
• Monitor and evaluate conversion trends in electronic referral systems, identifying opportunities for improvement.
• Design, deliver, and oversee training for electronic referral systems, CRM, and Dart Chart.
• Implement guidelines and coordinate training/educational programs for Admissions teams.
• Provide performance feedback, training support, and assist with related projects and initiatives.
• Complete and submit required reports and documentation accurately and timely.
• Support onboarding and orientation of new Admissions team members and designees.
• Represent Diversicare and its centers with professionalism, integrity, and service excellence.
• Uphold Diversicare’s Mission, Values, and Service Standards in all activities.
• Perform other related duties as assigned.
• 5 years’ experience in admissions/sales in the healthcare field
• Bachelor’s Degree or commensurate skilled nursing and LTC experience
• Advanced computer skills required, reports, analytics, training systems and communication
• Ability to travel required – some overnight (approximately 60-70%)
• Must be able to cover travel expenses (airfare, hotel, car, meals, rental, etc.) up front; company will reimburse in accordance with travel policy.
