What You’ll Do

• Be the Clinical Lead: Perform comprehensive assessments and manage acute and chronic conditions in complex, medically fragile populations.
• Drive Innovation: Utilize telehealth and evidence-based strategies to enhance patient outcomes and improve the overall care experience.
• Collaborate & Connect: Act as the vital liaison between hospitals, families, and facility leadership. You’ll be the "go-to" expert for nursing staff and therapy teams.
• Focus on Value: Champion quality metrics and cost-effective medicine that prioritizes the patient’s goals over everything else.

What You Bring to the Team

• The Credentials: A Master’s or Doctorate from an accredited program (NP or PA) with a current state license, Board Certification, and an active DEA.
• The Experience: A background in Geriatrics, Post-Acute, or SNF settings is strongly preferred. You understand the unique rhythm of the post-acute environment.
• The Mindset: You are independent yet collaborative, a master of time management, and a compassionate communicator who treats every patient with profound respect.
