The Traveling Social Services Specialist is responsible for assessing and supporting the emotional, financial, spiritual, educational, and social needs of residents/patients and their families, and for enhancing quality of life through individual, group, and interdisciplinary interventions. This role requires extensive travel to assigned centers to provide consistent, on-site support and ensure continuity of services across locations.

Responsibilities

  1. Assists in pre-admission and admission process with customers; aids clients in adjusting to nursing home placement
  2. Interviews residents and families to obtain relevant past and present adjustments, behavior and lifestyle.
  3. Documents progress and updates plans of care at the interdisciplinary care conference.
  4. Arranges transportation, makes appointments and acts as a liaison between all departments, residents’ families and outside agencies.
  5. Assists in handling and accepting placement through one-on-one counseling and group work.
  6. Ascertains potential and develops discharge plans when clients are admitted
  7. Arranges for post discharge services and follow-up care.
  8. Assesses emotional, financial, spiritual, educational and social needs of residents; ensures those needs are Manager, Education & Training by the facility or through the use of outside agencies.
  9. Acts as resident/family advocate and ensure the resident is knowledgeable in and exercises his/her rights.
  10. Responsible for organizing and conduction of Family Counsel Meetings and maintaining minutes of the meeting
  11. Maintains effective communication with residents, families, health care professionals and facility staff.
  12. Assist with training and onboarding of new hires.
  13. Other job duties as assigned by Supervisor.

Qualifications

  • Bachelor’s degree in Social Work required; must hold and maintain an active Social Work license in good standing, in accordance with all applicable state licensing requirements..
  • Minimum of one (1) year of supervised social work experience in a healthcare setting, working directly with individuals.
  • Demonstrated ability to build and maintain professional relationships and collaborate with external agencies to address health and welfare needs. 
  • Ability to work effectively with diverse populations in a variety of settings, including community and long-term care environments.
  • Strong critical thinking skills with the ability to evaluate case information and make informed decisions regarding appropriate interventions and procedures.
  • Prior experience in long-term care required
  • Must be able to travel up to 100% to assigned centers, including travel by car or air.
